Registration & Payment

I give permission for my child to be a member of the above school sport aerobics program coordinated by Industrie 1. I understand the commitment my child needs to make, to be a part of their team. I understand that payment is required prior to my child participating in this activity. I am also aware that my child is required to purchase a team costume for competitions and that there are further costs associated if my child’s team qualify for the National Championships in August, that are not included in the upfront fees.

I recognise that participation in this sport aerobics program will require physical fitness and I am satisfied that my child has the ability required to safely participate in this class. I consent for my child to receive any medical treatment required (including transport by ambulance), should they be injured or become ill during the class. I understand that although Industrie 1 and its service providers attempt to minimise any risk of personal injury within practical boundaries, accidents do happen, and all physical activities carry the risk of personal injury. I understand that Industrie 1 can except no responsibility for injury or illness resulting through class participation or my child’s decision to attempt moves/skills that are dangerous or beyond their ability. I agree that my child is physically capable to participate in this class and I have disclosed any medical conditions that may be of concern to the teachers of the class. I understand that Industrie 1 can take no responsibility for students outside of their scheduled class times.

I give Industrie 1 Sport Aerobics permission to use images/videos of my child in the course of their training and competitions, for purposes associated with the promotion of Industrie 1. Including but not limited to, the form of newsletters, website, social media, and in print and other visual media.
